Z table limit mount file?

Is there a cut file for a mount that limit switches for the z table would screw to? I remember seeing somewhere on here that someone had made one but can find the reference or file download. I believe it was made from acrylic but don't quote me on it.

The thread there has the various mount and button options if you are using the SC04.. if using the DSP, just the limit switches option works fine.
