How did this happen?

I am wondering how the state of my laser tube happend (see pic attachment) and how I can prevent a new laser tube from doing the same. I have about 100 hours on the laser but have been using it very frequently and for long periods of time in the last 3 weeks. I've monitored my water temps and the water never reached more than 80 degrees fahrenheit.

Help me out here, and thanks in advance

I think I have figured it out. When I pulled the tube out and emptied the contents, there appears to have been an air bubble in the cap piece, so the backside of the mirror and the tube was cooled, but the cap piece was not. It appears the cap piece got hot enough to melt the adhesive and then fell off. I think I might be able to repair this, but am buying a replacement tube as we speak.

I think you should pay attention to the flow rate. The small piece of tubing seems clogged. You need to make sure it is straight and with decent flow rate

I had that happen to a new tube when I was installing..while attaching the water line,that part came off...I glued it back on with some 2 part epoxy and it worked fine.
